Online Team Registration

You may register for the Golf Tournament by Fax, Mail, or Paypal.

To register online:
Each team should be registered by the Team Captain. The Team Captain should complete the form below and click the "Register" button. You will be taken to the Paypal page, where you can finalize your payment. After submitting your registration fee, please complete the Registration form (adobe reader required) and mail or fax it to Hospice Support Care.

To register by mail or fax:
Complete the Registration form and mail or fax it to Hospice Support Care. If you are mailing your registration, please submit the fee with your registration form. If you are faxing your registration, please mail your fee separately. If you are mailing your fee and faxing your registration separately, please ensure that your team captain's name is listed somewhere on the check.
